Dr. Fred Burbank is a physician. He was trained in sleep medicine by the man who more or less founded the field. And he has restless legs himself.

He tells the story on camera, in a lecture he gave at a medical centre. He was watching television one evening when the ads for the RLS drugs started running, and he turned to his wife and said: you know honey, I've got that.

Growing pains as a kid. A quiet stretch in his twenties. Then it came back in his forties and he assumed he was just getting old.

He started by asking a room full of people what already works

Halfway through that lecture, Burbank stops presenting and asks the audience a question. Everyone there has RLS. He wants to know what they actually do when it starts at night.

The answers come fast. One woman runs on the spot. One lies on her arm to make it go numb. One gets up and quilts. One goes downstairs for a glass of orange juice and back up again, several times a night. One says, plainly, that she hits her leg as hard as she can for ten minutes.

Then he says the thing that reframes the entire condition:

"All of these methods are ways to have something you can do in the face of an attack. But they're all incompatible with sleeping."

That's the good news hiding inside the bad news. Things already work. Walking works. Movement works. Standing up works, reliably, for almost everyone.

The only thing wrong with all of them is that they require you to be awake.

So the question was never "what helps." It was: can you get the same relief without leaving the bed?

The hammer and your thumb

Burbank explains the underlying idea with something everyone in the room had done.

What's the first thing you do? You shake your hand. Hard. Nobody stands still and stares at their thumb, and in the lecture he points out why: if you did, it would hurt more. Researchers call it counterstimulation. Burbank's whole idea was to find a version of it that worked while you were lying still, so your legs could get what they're asking for without you having to walk the hallway to deliver it.

Then he tested it properly

He and his partner rented a few small workshops, built a bedroom inside one of them, and advertised in the local paper for people with RLS scoring 15 or higher on the international scale.

He then worked with Dr. Mark Buchfuhrer, a sleep specialist affiliated with Stanford, and Dr. Branko Kopjar of the University of Washington, on two randomized, double-blind clinical studies. It wasn't a quiet announcement either. The American Academy of Sleep Medicine, the professional body for sleep physicians in the United States, reported the clearance to its members at the time.

What "FDA cleared" actually means

It means somebody with authority looked.

In the United States, a company cannot sell a device for a medical purpose on its own say-so. It has to submit its evidence to the Food and Drug Administration, state in writing exactly what it claims the device does, and wait. The agency reviews the file and either permits that specific claim or refuses it.

And when it clears, the wording isn't marketing language. It's the agency's own, and it's deliberately narrow. In this case: to improve the quality of sleep in patients with primary restless legs syndrome, through the use of vibratory counterstimulation.

Burbank says in the lecture that it took about five years. The agency had no existing category to file it under, because nothing like it had come through before, so they had to build one.

Most things sold for restless legs have never been through any of that. Supplements, creams, socks, plates: they're sold as wellness products, and wellness products require no submission, no evidence, and no permission from anyone. That's not an insult, it's just the category they're in.

The difference isn't how confident the marketing sounds. It's whether anyone with authority ever looked at the file.

The comparison that surprised him

Burbank then did something most device makers avoid. He compared his two trials against every published trial of the RLS drugs the FDA had approved, and looked at the sleep improvement side by side.

In the lecture he calls the result astounding, and says he did not expect it. The vibration produced roughly the same improvement in sleep as the approved drugs, without drug-like side effects.
